Polymyositis

PolymyositisPolymyositis is an autoimmune disease characterized by muscle weakness and myalgia. The main clinical manifestations are diffuse muscle inflammatory disease characterized by symmetric proximal limbs, neck muscles, and pharynx muscle weakness, muscle tenderness, and increased serum enzymes. Most of the onset is subacute, which can occur at any age. It is more common in middle-aged and older, and slightly more in women. Some patients have malignant tumors before the disease, and about 20% of patients have other autoimmune diseases such as lupus erythematosus, scleroderma, rheumatoid arthritis, Sjogren’s syndrome, etc. Early diagnosis and treatment of polymyositis are very important. As long as the hormone or immunosuppressive treatment is applied in time, the prognosis of polymyositis alone is expected to be good, while the prognosis of patients with a malignant tumor and a variety of connective tissue diseases is poor.

Diagnosis of Polymyositis

There are three detection methods for polymyositis:

  • Blood test. The white blood cell count in the peripheral blood increases during the acute phase, and is normal during the stable phase. ESR can be accelerated. Serum creatine kinase and lactate dehydrogenase are significantly increased during disease activity.
  • Electromyography. It suggests that myogenic and neurogenic lesions coexist, and spontaneous fibrillation potentials and positive phase spikes increase.
  • Muscle biopsy. Pathological changes such as muscle fiber degeneration, necrosis, muscle atrophy and regeneration, muscle fiber interstitial inflammatory cell infiltration, small blood vessel obstruction, and capillary endothelial hyperplasia may be detected.
