Leishmaniasis

LeishmaniasisLeishmaniasis is a chronic endemic disease caused by Leishmania, also known as kala-azar. The pathogen of leishmaniasis is stored in domestic dogs. It is then transmitted from dogs to humans and becomes a zoonotic infectious disease. It can also be transmitted by sandfly bites. The clinical features of the disease are mainly manifested as long-term irregular fever, splenomegaly, anemia, weight loss, decreased white blood cell count, and increased serum globulin. In addition, the patient also develops pigmentation on the skin of the face, abdomen, hands, and feet. Without appropriate treatment, most patients will die within 1-2 years due to complications with other diseases. The disease mostly occurs in Mediterranean countries, tropical and subtropical regions.

Diagnosis of Leishmaniasis

There are several ways to test leishmaniasis:

  • Blood routine. The whole blood cells are reduced, among which white blood cells and platelets are significantly reduced. While the serum globulin is increased.
  • Serum antibody test. Using ELISA, indirect hemagglutination test, convection immunoelectrophoresis, indirect fluorescence test and direct agglutination test to detect the serum antibodies of Leishmania, the positive rate and false positive rate is high. In recent years, the use of molecular biology methods to obtain pure antigens has reduced the false positive rate.
  • Serum circulating antigen test. The monoclonal antibody antigen spot test for the diagnosis of leishmaniasis has high positive rate, good sensitivity, specificity, and reproducibility. Only a small amount of serum is required, and it can also be used for efficacy evaluation.
  • Molecular biology methods. The detection of Leishmania DNA in blood or bone marrow puncture specimens by PCR technology is highly sensitive and can identify protozoa species, especially for asymptomatic infections. Real-time PCR can detect protozoa quantitatively and evaluate the early curative effect.
