Primary Sclerosing Cholangitis

Primary Sclerosing CholangitisPrimary sclerosing cholangitis is a chronic cholestatic autoimmune disease, which is more common in young men, and is often associated with inflammatory bowel disease, especially ulcerative colitis. Its main feature is progressive cholangitis and fibrosis, which can cause scars in the bile duct. These scars harden and narrow the bile ducts and gradually cause severe liver damage. Most patients eventually develop liver cirrhosis, portal hypertension, and liver decompensation. The pathogenesis of primary sclerosing cholangitis is still unclear, and there is a lack of effective treatments in clinical at present. The focus of primary sclerosing cholangitis care is to monitor liver function, manage symptoms, and if possible, perform surgery to temporarily open the obstructive bile duct.

Diagnosis of Primary Sclerosing Cholangitis

The diagnosis methods of primary sclerosing cholangitis mainly include the following three:

  • Serological testing. The majority of patients have elevated serum APL, usually 3-5 times the normal level, which is the most typical biochemical abnormality of the primary sclerosing cholangitis. In addition, patients may be accompanied by fluctuating bilirubin levels, elevated serum transaminase and IgG. A variety of autoantibodies can be detected in patients' serum, including ANCA, ANA, SMA, etc.
  • Imaging diagnosis. Endoscopic retrograde cholangiography (ERC) is the gold standard for diagnosing primary sclerosing cholangitis. Magnetic resonance cholangiography (MRC), CT and other methods can also assist in the diagnosis of the disease, showing thickening, expansion, and stenosis of the bile duct wall.
  • Histological examination. The liver biopsy can help determine the extent of liver damage. This test should only be used as long as the diagnosis of primary sclerosing cholangitis is still uncertain after a minimally invasive test.
