Diabetes

Diabetes is a disorder of carbohydrate, protein, and fat metabolism caused by insufficient insulin secretion or insulin utilization disorder, with hyperglycemia as the main sign. The typical clinical manifestations of diabetes include polydipsia, polyuria, polyphagia, weight loss, high blood sugar, and glucose in urine. Too long a course of the disease can cause multi-system damage, leading to chronic progressive disease, hypofunction and failure of the eyes, kidneys, nerves, heart, blood vessels and other tissues and organs. Serious diabetes can cause acute serious metabolic disorders. Diabetes is an major cause of cardiovascular and cerebrovascular diseases, death, amputation, blindness, renal failure and heart failure, and it is a common disease. At present, the prevalence and incidence of diabetes are rising sharply worldwide.

Diagnosis of Diabetes

The diagnosis of diabetes is mainly based on the following indicators:

  • Urine sugar. When the glucose in the blood exceeds the capacity of the kidneys, it will appear in the urine. A positive result is an important clue to finding diabetes, but it cannot be used as a basis for diagnosis.
  • HbA1c and GA. Glycated hemoglobin A1c (HbA1c) and glycated albumin (GA) are used to reflect the average blood glucose level over time. HbA1c reflects the blood glucose level in the last 2-3 months; GA reflects the average blood glucose level in the last 2-3 weeks.
  • Pancreatic β-cell function test. Including insulin release test and C-peptide release test, which reflect the ability of islet cells to release insulin mediated by glucose.
  • Blood lipids. Dyslipidemia is common in diabetic patients, especially when blood sugar is poorly controlled. It is manifested as increased levels of triglycerides, total cholesterol, low-density lipoprotein cholesterol, and decreased high-density lipoprotein cholesterol.
  • Immune indicators. Islet cell antibody (ICA), insulin autoantibody (IAA) and glutamate decarboxylase (GAD) antibody are three important indicators of type 1 diabetes humoral immune abnormalities. Among them, GAD antibody has high positive rate and long duration, which is of great diagnostic value for type 1 diabetes.
