Group A Streptococcal Infection

Group A Streptococcal InfectionGroup A Streptococcus (GAS), also known as Streptococcus pyogenes, is one of the most important pathogens in human bacterial infections. GAS can invade any part of the body, but the upper respiratory tract infections are the most common, followed by skin and soft tissue infections. GAS infection can cause two types of diseases: suppurative disease and non-suppurative complications. GAS infection is also an indirect cause of allergic diseases rheumatic fever and acute glomerulonephritis. At present, the transmission of streptococcal infections through foods such as milk or dairy products contaminated with streptococcus is rare. Invasive group A streptococcal infection manifests as toxic shock syndrome (TSS) and is often life-threatening.

Diagnosis of Group A Streptococcal Infection

There are several methods to detect group A streptococcal infection:

  • Blood routine. The total number of white blood cells and neutrophils is increased, and those with purulent complications are higher. In severely infected patients, such as TSS patients, the cell classification may shift to the left.
  • Urine routine. Patients with high fever may have proteinuria. When nephritis is complicated, urine protein increases, and red blood cells appear. Uncomplicated urine abnormalities disappear after the fever subsides.
  • Etiological test. For the etiological test of patients with acute pharyngitis and tonsillitis, throat swab culture should be performed first. Then immunological methods should be used to detect bacterial antigens in the samples. This method is the gold standard for the diagnosis of group A streptococcal pharyngitis or tonsillitis.
