Hepatitis D

Hepatitis DHepatitis D virus (HDV) is a defective negative-strand RNA virus, which must rely on hepatophilic DNA viruses such as hepatitis B virus to provide a shell for it to replicate. Therefore, hepatitis D is an infectious disease caused by HDV and HBV. The disease has a worldwide distribution, especially in southern Italy is highly endemic. The developing countries have a higher rate of virus infection, which has the basis for causing pneumonia. It is mainly spread through blood transfusions and blood products, similar to the way hepatitis B is spread. The superinfection of HDV and HBV can promote liver damage to aggravate and easily develop into chronic active hepatitis, cirrhosis and severe hepatitis.

Diagnosis of Hepatitis D

The specific diagnosis method for hepatitis D is the detection of HDAg or HDV-RNA from the liver and serum, or the detection of anti-HD and anti-HD IgM from the serum, both of which are used as the basis for diagnosis:

  • HDAg. Use immunoenzymatic or radioimmunoassay to detect serum HDAg, if positive, it is helpful for early diagnosis. Intrahepatic HDAg can be detected by immunofluorescence or immunohistochemistry. It can be found that HDAg is located in the liver nucleus or in the hepatocyte plasma, which is beneficial to diagnosis.
  • Anti-HD and anti-HD IgM. The immunoenzyme-linked method or radioimmunoassay can be used to detect serum anti-HD. High-titer anti-HD IgM is a marker for the diagnosis of acute hepatitis D, and anti-HD IgG is the main marker for identifying chronic hepatitis D. At present, the detected anti -HD, in fact, mainly IgG type.
  • HDV-RNA. HDV-RNA is detected by in situ hybridization or transfer hybridization. HDV-RNA positivity is direct evidence of HDV replication and HDV infection. Reverse transcription-polymerase chain reaction (RT-PCR method) can be used to detect serum HDV-RNA.
