Primary Biliary Cirrhosis

Primary biliary cirrhosis is an autoimmune disease in which the bile ducts in the liver are slowly destroyed. Bile is a fluid produced in the liver. It helps digestion and helps your body get rid of cholesterol, toxins and old red blood cells. When the bile ducts are damaged, bile may return to your liver, causing irreversible scarring of liver tissue (cirrhosis). Primary biliary cirrhosis often coexists with other immune diseases such as rheumatoid joints, Sjogren’s syndrome, scleroderma, Hashimoto’s disease, etc., and occurs mostly in middle-aged women. More than half of patients with primary biliary cholangitis have no obvious symptoms at the time of diagnosis. The disease can be diagnosed when blood tests are performed for other reasons.

Diagnosis of Primary Biliary Cirrhosis

There are several diagnostic methods for primary biliary cirrhosis:

• Blood test. Blood lipids, serum bile acid, conjugated bilirubin, AKP and GGT and other enzymes are significantly increased, and transaminase is normal or slightly increased.
• Urine test. Urine bilirubin is positive, and urobilinogen is normal or decreased.
• Antibody test. A blood test can be done to check for anti-mitochondrial antibodies (AMA). These substances rarely occur in people without primary biliary cirrhosis, even if they have other liver diseases. Therefore, a positive AMA test is considered a very reliable sign of the disease. However, a small number of patients with primary biliary cirrhosis do not have AMA.
• Imaging diagnosis. Ultrasound, ERCP, CT, PTC and other imaging methods are used to understand whether there are diseases of intra- and extrahepatic bile duct dilation and extrahepatic obstructive jaundice.
