Leptospirosis

LeptospirosisLeptospirosis is an acute zoonotic infectious disease caused by pathogenic Leptospira. The main manifestations are blood emia in the early stage, various organ damage and dysfunction in the middle stage, and various allergic symptoms in the late stage. In severe cases, there are obvious liver, kidney, central nervous system damage and diffuse lung bleeding, even life-threatening. Rodents and pigs are the main host and source of infection. They are mainly spread through contact with animal excrement containing Leptospira or their contaminated water, including infection by eating food and water contaminated by rat urine. Infection after rat and dog bites has also been reported, with general human susceptibility.

Diagnosis of Leptospirosis

The following methods can be used to detect leptospirosis:

Physical examination. Including superficial lymph node examination, skin and mucous membrane examination, lung physical examination, abdominal physical examination, cardiac examination, nervous system examination, etc.

Blood routine. The leucocytes and neutrophils increased slightly.

Urine routine. There is proteinuria. Red blood cells and white blood cells are also visible under a microscope.

Immune test. Leptospira-specific antibodies are tested by micro-agglutination test and ELISA method, and they are usually positive one week after the illness.

Blood culture. Blood samples are collected within one week after the onset of the disease, and inoculated in Korotkoff medium. Under appropriate conditions, Leptospira can be cultured.

Cerebrospinal fluid examination. Patients with meningitis will experience increased cerebrospinal fluid pressure, increased protein, and decreased white blood cells.

Imaging tests. X-ray is the main form. In patients with pulmonary hemorrhagic blood type, ground-glass or diffuse spot/sheet or fused sheet opacity can be seen in both lungs on X-ray.
