Mumps

MumpsMumps refers to an acute self-limited respiratory infectious disease caused by the infection of the mumps virus, spread through droplets. Mumps mainly occurs in children and adolescents, and is characterized by non-purulent swelling and pain of the parotid glands. In addition to the parotid gland, the mumps virus can also invade the nervous system and other glandular tissues, causing meningitis, oophoritis, myocarditis and other diseases. Mumps outbreaks are mainly concentrated in densely populated areas. However, the mumps virus antigen is relatively stable, and no significant mutation has been found. After infection, lifelong immunity can generally be obtained, re-infection is very rare. Humans are the only natural host of the mumps virus.

Diagnosis of Mumps

There are several methods to test mumps:

  • Determination of amylase in serum and urine. About 90% of patients in the early stage of the disease have elevated blood and urine amylases, and the level of elevation is generally proportional to the degree of parotid gland swelling, which is helpful for doctors to diagnose.
  • Immunological and serological testing. Early diagnosis can use specific antibodies or monoclonal antibodies to detect mumps virus antigens. Specific antibodies can usually be detected after the second week of the disease course. Use complement fixation test and hemagglutination inhibition test to detect antibodies. The antibody titer in the recovery phase is 4 times or higher than that in the acute phase.
  • RNA detection of mumps virus. The polymerase chain reaction (PCR) technology is used to detect the RNA of mumps virus. The sensitivity and specificity are very high, which can significantly improve the diagnostic rate of suspicious patients. The saliva, cerebrospinal fluid, and urine of patients within 3-8 days of onset can be sampled for testing.
  • Virus isolation. Mumps virus can be diagnosed by separating the mumps virus from the patient's saliva, blood, urine, cerebrospinal fluid and other samples in the early test.
