Mononucleosis

Mononucleosis is an acute self-limited infectious disease caused by a contact infectious virus, Epstein-Barr virus. The clinical features are fever, pharyngitis, hepatosplenic lymphadenopathy, a significant increase in peripheral blood lymphocytes and abnormal lymphocytes, a positive Heterophil agglutination test, and the appearance of anti-EBV antibodies in the body after infection. The course of the disease is often self-limited. Most patients have a good prognosis, and a few have serious complications such as hemophagocytic syndrome. Adolescents and young adults are more likely to develop the disease, and close oral contact is the main way of transmission, such as kissing, sharing utensils, and droplet transmission is also possible.

Diagnosis of Mononucleosis

There are several methods to test mononucleosis:

• Blood test. The total number of white blood cells is normal or slightly increased, and mononuclear cells (lymphocytes, monocytes, and atypical lymphocytes) can reach more than 60%.
• Heterophil agglutination test. This is a standard diagnostic test for mononucleosis. The principle is that the patient's serum often contains heterophilic antibodies belonging to IgM that can agglutinate the red blood cells of sheep or horses. This test is positive in the early stages of the disease.
• EBV antibody test. Anti-VCA IgM and IgG are more commonly used. VCA IgM appears early, disappears quickly, has high sensitivity and specificity, and has early diagnostic value. EA IgG is suitable for epidemiological investigations.
• EBV DNA test. Test whether the Epstein-Barr virus DNA in blood, saliva, oropharyngeal epithelial cells, urine or tissues is positive.
