Antiphospholipid Syndrome

Antiphospholipid SyndromeAntiphospholipid antibody syndrome (APS), also known as Hoghes syndrome, is a disease of the immune system. APS occurs when the immune system mistakenly produces antibodies that make blood more likely to clot, causing dangerous blood clots in the legs, kidneys, lungs, and brain. In pregnant women, APS can also lead to miscarriage and stillbirth. APS can be secondary to systemic lupus erythematosus or other autoimmune diseases, but it can also occur alone (primary APS). Regardless of primary or secondary APS, there is no difference in clinical manifestations and laboratory tests. The incidence of women is significantly higher than that of men. There is no cure for APS, but medication can reduce the risk of thrombosis.

Diagnosis of Antiphospholipid Syndrome

The main clinical symptoms of APS are recurrent thrombocytopenia, thrombosis, and repeated preterm labor, while laboratory tests mainly include positive phospholipid antibodies such as anticardiolipin antibodies. For patients with both clinical symptoms and phospholipid antibody positive, it can be diagnosed as APS. The detection and diagnosis methods of APS are as follows:

  • Routine test. Blood and urine routine tests, liver and kidney function, electrolytes, complement as well as erythrocyte sedimentation rate tests.
  • Antibody detection. Anti-phospholipid antibodies, anti-cardiolipin antibodies, APL antibodies such as LA and ACL antibodies are positive. The International Standardization Symposium has formulated a unified ELISA method to detect APL, which can be used to quantitatively or semi-quantitatively determine IgG, IgA, and IgM of APS.
  • Imaging diagnosis. Both vascular ultrasound and angiography are used to diagnose thrombosis in larger arteries or veins. MRI is beneficial to the diagnosis of major vascular diseases such as the aorta and vena cava.
  • Histopathological biopsy. Used to identify inflammatory vascular occlusion.
